Forms

Forms is where your clinic creates, manages, and stores the templates used across cases.

Veterinary clinics use forms for everything from hospitalisation records and dental charts to surgery consents and other documents that need to be completed or signed by a pet parent.

smollOS comes with built-in forms and lets your clinic create its own custom templates.

Built-in forms

smollOS includes three native forms designed around common veterinary workflows:

  • Patient Form

  • Dental Chart

  • Surgery Consent

  • Euthanasia Consent

Some forms are designed purely for clinical records. Others can be sent to the pet parent for review and signature.

Patient Form

The Patient Form is designed for hospitalised pets.

It works like the physical patient sheets traditionally placed on a hospital cage, giving the veterinary team a simple record of what happens throughout the pet's stay.

The form can include a weekly view of the hospitalisation period, with space to record activities and care performed each day.

When added to a Case, smollOS automatically populates the pet's information.

The form can then be exported as a PDF.

The Patient Form does not require a pet parent signature.

It is primarily a clinical and operational record.

Dental Chart

The Dental Chart is a digital record for documenting dental procedures.

The veterinary team can interact with the chart to mark teeth that have been:

  • Extracted

  • Pulled

  • Cleaned

  • Polished

  • Identified for removal

  • Otherwise treated

The chart is associated with the Case and can be saved, exported as a PDF, or shared with the pet parent.

The dental history can also carry forward between dental visits, allowing the veterinary team to see what was previously done.

Consent forms

smollOS includes generic templates for common consent workflows, including:

  • Surgery Consent

  • Euthanasia Consent

These forms are designed to be completed and signed by the pet parent.

When a consent form is added to a Case, smollOS automatically populates available information such as the pet, pet parent, clinic, and date.

The clinic can then send the form to the pet parent.

Sign from your phone

Forms that require a signature can be completed remotely.

The clinic can send a form directly to the pet parent, or provide access through a QR code.

The pet parent opens the form on their phone, reviews the information, and signs it digitally.

Once completed, the signed form is stored against the relevant Case.

This means there is no need to print, scan, or manually upload signed documents.

Create a template

Every clinic has its own workflows and paperwork.

With New Template, you can create your own forms for whatever your clinic needs.

Start with:

  • Template title

  • Body

  • Signature requirement

Dynamic information

Templates can use placeholders to automatically pull information from the Case and clinic.

Available placeholders can include:

  • Pet name

  • Pet parent name

  • Clinic name

  • Species

  • Breed

  • Date

  • Pet parent phone

  • Pet parent email

For example, instead of manually entering the pet's name every time, you can use the Pet Name placeholder.

When the form is generated inside a Case, smollOS replaces the placeholder with the relevant information.

Signature required

When creating a template, you can decide whether the form requires a pet parent's signature.

Signature required: The form can be sent to the pet parent for digital signing.

No signature: The form is used as a clinical or operational record.

This makes the same template system flexible enough for both internal documentation and client-facing forms.

Forms inside a Case

Once a template exists, it becomes available across Cases.

From the Forms tab inside a Case, the veterinary team can select the relevant template.

smollOS creates the form using the Case information, so the pet and pet parent details are already populated.

From there, the team can complete it, send it, request a signature, generate a PDF, or store it as part of the Case record.

One template. Every Case.

Create your forms once and use them wherever they're needed.
